Purpose and audience

The app and website versions of this patient and carer toolkit provide information and tools to support:

  • Self-management by people taking multiple medicines.
  • Shared decision-making by patients and healthcare professionals during consultation and medicines review. This includes supporting patients to prepare in advance of meeting with their healthcare professional, and to follow up on key issues after the meeting.

Privacy statement

This app collects information the you provide about the impact of your medicines and medicines review. This includes your name and date of birth and your Study ID if you are part of the i-SIMPATHY study.

Mobile app 

The information is stored on your mobile device and it is your responsibility to apply your own security measures to your device  – e.g. password protection.  If you choose the “Share” option within the app, this information is emailed securely in encrypted form to your chosen healthcare professional. The healthcare professional will review it and store it securely with your other health records. You have the right to request access at any time to see how this information is stored and can ask that the information is amended or removed. Your local practice will provide details of how you can request access to your health records.

If you provide your iSIMPATHY study ID when you choose the "Share" option within the app, the information you provide is also emailed securely in encrypted form to a secure database held by Scottish Government. Any information that could identify you personally will be excluded from the upload process, so that it is completely anonymous.

Web form

The 'Questions for my review' form on the website version of the Manage Medicines app uses questionnaire software called Webropol, provided by NHS Greater Glasgow and Clyde. Your responses are stored securely on a UK server in compliance with the General Data Protection Regulations.

If you provide your iSIMPATHY study ID when you complete the form, a member of healthcare staff transfers your responses  securely by email  to a secure database held by Scottish Government. This enables analysis of the impact of medicines review as part of the European iSIMPATHY project. Any information that could identify you personally will be excluded from the upload process, so that it is completely anonymous.

If you choose to email the report of your responses in the web form to a healthcare professional, the report will be sent by secure email. The healthcare professional will  review it and store it securely with your other health records. You have the right to request access at any time to see how this information is stored and can ask that the information is amended or removed. Your local practice will provide details of how you can request access to your health records.

If you are unable to complete the responses to the questions on the app yourself, your healthcare professional may use the app to enter the responses in consultation with you. If the healthcare professional is using their own device, they are required to delete the record of responses after emailing it to an NHS email address.

Why this app and website were developed

The Scottish Government Polypharmacy Guidance emphasises the need to improve care and support for the growing number of people with multiple conditions and taking multiple medicines.

The guidance highlights the need to make decisions about medicines based on outcomes that matter to the individual patient. To make this possible, we need to enable people to take a more active role in managing their medicines and the need to develop a shared decision-making approach with patients and professionals working as partners. This app and website have been developed to support these goals of self-management and shared decisions for people taking multiple medicines.

The app and website have been developed with extensive input from patients, carers and professionals through:

  • Feedback on patient and shared decision-making sections of the original polypharmacy app launched in 2018.
  • A review of the research evidence and a series of interviews with patients and professionals, to identify the most important outcomes for people taking multiple medicines.
  • A survey of patients and carers in two health boards – NHS Lanarkshire and NHS Ayrshire and Arran – to confirm priorities for a mobile app and website and to identify how best to support people who are not confident in using digital tools for health information.
  • Five workshops with combined patient and professional groups to gather input to development of the app and website and to capture feedback on initial mock-ups.

Updating

  • The Scottish Government Effective Prescribing and Therapeutics Branch will review content within the app annually and identify any requirements for updating.
  • Medicines information content extracted from NHS UK is updated as a minimum every 7 days.
  • Technical maintenance and updating of the app and website are carried out by Tactuum Ltd and funded through the national decision support programme led by the Digital Health and Care Institute.
